Care Manager

The Care Manager plays a key role in ensuring individuals with developmental and intellectual disabilities and autism receive coordinated, responsive, and high-quality supports. By guiding and mentoring a team of Service Coordinators, the Care Manager strengthens service delivery, promotes consistency, and helps people access the resources they need to live fuller, more independent lives. Through close collaboration with internal teams, families, and community partners, the Care Manager helps build seamless systems of care that prioritize continuity, accountability, and positive outcomes for the individuals and families served every day.

Job Duties:
  • Provide individual and group supervision to Service Coordinators.
  • Provide onboarding, training, and guidance in person-first and client-centered care principles.
  • Monitor, analyze, and report on performance and financial metrics.
  • Deliver and facilitate training on electronic medical records systems, behavior support plans, and other skills important to Coordinators and direct care providers.
  • Be a resource during crises, including requesting and utilizing other teams as needed.

Qualifications, Knowledge, Skills:
  • High School completion or equivalent; BA preferred.
  • Team leadership experience.
  • Three or more years providing services to individuals with developmental and intellectual disabilities and autism.
  • Valid driver's license and consistent access to a safe, reliable, insured vehicle.
  • Knowledge of Behavioral Support Plans, Communication Plans, and Individual Support Agreements.
  • Demonstrated knowledge of positive regard for individuals with mental health concerns, substance use and co-occurring disorders.
  • Demonstrated ability to collaborate with internal and external partners.
  • Demonstrated strong communication and de-escalation skills.

Working Conditions:
  • Requires working in a variety of indoor and outdoor requirements, including office spaces, all program spaces, in the community, and in client living environments.
  • Work hours may include evening and weekend shifts, as required to meet program staffing requirements.
  • Working with individuals who may present challenging behaviors requires safety planning, crisis intervention, and de-escalation skills. 
 
Compensation and Benefits:
This full time, exempt job starts at $26.33/hr and includes comprehensive benefits including paid time off, employer-sponsored health, dental, life, and disability insurances; a 403(b) retirement plan with generous employer match; and educational benefits such as tuition reimbursement and student loan repayment.
